Is your cholesterol test missing a number?

There's a second cholesterol number, and standard panels skip it. Nearly a third of members with normal LDL had it raised.

Ultrahuman looked at 4,841 members carrying both LDL cholesterol and apolipoprotein B (ApoB) on their most recent Blood Vision panel, drawn between August 2025 and July 2026. Among the 3,120 whose LDL cholesterol came back in the normal range, nearly one in three carried a raised ApoB.

A normal LDL cholesterol does not mean a normal second number. Of the 3,120 Ultrahuman members whose LDL cholesterol sat below 130 mg/dL, 966 of them, 31.0%, carried an ApoB at or above 90 mg/dL on the same blood draw. That is close to one member in three leaving a clean-looking result with a second marker their panel never reported.

The numbers

Two markers, one blood draw, and they do not always agree.

  • Of the 3,120 members whose LDL cholesterol was below 130 mg/dL, everything US guidance counts short of borderline high, 966 of them, 31.0%, had an ApoB at or above 90 mg/dL.
  • Read against the whole cohort of 4,841 members, that group is 20.0%.
  • 44.5% of members sat below both lines, and 35.6% had a raised LDL cholesterol.

What does ApoB measure that LDL cholesterol does not?

ApoB counts particles. LDL cholesterol weighs their cargo. Very low density lipoprotein, LDL, chylomicron and lipoprotein(a) particles each contain one apolipoprotein B molecule [1], so the two markers answer different questions: how many particles there are, and how much cholesterol they carry between them. The load each one takes varies. Sniderman and colleagues, in JAMA Cardiology, put it plainly: “Cholesterol can only enter the arterial wall within apoB particles” [1]. So you and the next person can hand in the same LDL cholesterol and carry very different numbers of particles.

Among 389,529 UK Biobank adults, median age 56, with no heart disease and on no cholesterol-lowering treatment, ApoB, non-HDL cholesterol and triglycerides each tracked heart attack risk on their own. So the researchers compared people who matched on two of them and differed on the third. Only ApoB held up: at the same ApoB, a higher non-HDL cholesterol or triglycerides no longer meant higher risk [2].

How often does a normal LDL cholesterol come with a raised ApoB?

Close to one member in three. Figure 1 puts the whole cohort on a single chart, so you can find where your own LDL cholesterol and ApoB would sit on it.

Read the chart leftwards from the vertical line and you have the group this article is about. Everyone left of it would be told their LDL cholesterol is fine, and roughly a third sit above the horizontal line with elevated ApoB. Nothing in the result hints at it: the members in that coral block hold a median LDL cholesterol of 117 mg/dL, inside the near optimal band. LDL cholesterol answers one question; ApoB answers the second on the same blood draw.

Where do those two lines come from, and why does moving them matter?

These two lines come from published reference ranges. US cholesterol guidance splits LDL cholesterol into bands: optimal below 100 mg/dL, near optimal from 100 to 129, and borderline high from 130 to 159. A line at 130 mg/dL is where a normal reading stops. For ApoB, clinical laboratories print the band on the report itself: below 90 mg/dL desirable, 90 to 99 borderline, 100 and above high.

The ApoB line is the stricter of the two. Among 12,688 US adults not on a statin, the typical ApoB was 80 mg/dL at an LDL cholesterol of 100 and 140 mg/dL at 190 [3], which puts an LDL cholesterol of 130 nearer an ApoB of 100. The 2018 US cholesterol guideline was retired and replaced in 2026, and a review of the new one reports selective ApoB testing and treatment goals returning for chosen cases [4].

What this analysis can't tell you

We are describing two numbers on one blood draw, not what happened to anyone after it. Nobody was followed and no heart condition was accounted for, so nothing here is a risk estimate. We do not know who was already on cholesterol-lowering treatment, and the panels come from many labs rather than one assay. The two lines are reporting conventions, not treatment goals, and they are not matched to each other. The choice of line moves the answer a long way: in one Polish survey the share of people whose two markers disagreed ran from 0.2% to 45.2%, on the same blood samples, depending only on where the lines were drawn [5]. Members who pay for a panel are more health-engaged than most, so this split may not hold outside this cohort. Median age is 37 among the 4,696 members with an age on file, and the cohort is 3,045 men, 1,448 women and 348 who did not specify.

What this means for you

Close to one member in three here had a normal LDL cholesterol alongside a raised ApoB. If your last panel reported only LDL cholesterol, the useful next step is asking for ApoB on the next one. And when you compare your own against a published threshold, check which one. Among the members whose LDL cholesterol was normal, 56.3% sat at or above an ApoB of 80 mg/dL, 31.0% at or above 90, and 14.2% at or above 100.

Frequently asked questions

How do I find out my own ApoB?

Look at your last panel and check whether ApoB is on it. A standard lipid panel reports total cholesterol, LDL, HDL and triglycerides. ApoB is usually not among them, so you have to ask for it or take a fuller panel. It is one more marker on the same blood draw, and most labs print the desirable band right next to the result.

Why can two people with the same LDL cholesterol have different ApoB?

Because the same amount of cholesterol can ride on a different number of particles. Small ones carry less each, so it takes more of them to hold the same total, and the count is what ApoB reports. Among 12,688 US adults not taking a statin, the middle 95% of ApoB results at an LDL cholesterol of 100 mg/dL still spanned 66 to 99 mg/dL [3].

Who does this not apply to?

Anyone already on cholesterol-lowering treatment, because both markers move under it and any gap you see is partly the treatment. It also describes members who chose to buy a panel, which is not a population sample. The closest comparison in the literature followed 27,533 initially healthy women and found 18.9% whose two markers disagreed, against median cut-offs rather than the fixed lines used here [6].
